around the same time was Adult BMI Assessment developed by NCQA. That measure was followed by a
pediatric measure, also developed by NCQA, called Weight Assessment and Counseling for Nutrition and
Physical Activity for Children/Adolescence.
The adult obesity quality measure from CMS, Preventive Care and Screening: Body Mass Index (BMI)
Screening and Follow-Up Plan, is defined as the “Percentage of patients aged 18 years and older with a
BMI documented during the current encounter or within the previous twelve months AND who had a
follow-up plan documented if the most recent BMI was outside of normal parameters ” (CMS, Explore
Measures & Activities, 2022). The other adult obesity quality measure from NCQA, Adult BMI
Assessment, is defined as “The percentage of members 18– 74 years of age who had an outpatient visit
and whose body mass index (BMI) was documented during the measurement year or the year prior to
the measurement year” (NCQA, Proposed Retirement for HEDIS MY 2020, 2020). The pediatric obesity
quality measure, Weight Assessment and Counseling for Nutrition and Physical Activity for
Children/Adolescents, is defined as the “Percentage of patients 3 -17 years of age who had an outpatient
visit with a Primary Care Physician (PCP) or Obstetrician/Gynecologist (OB/GYN) and who had evidence
of BMI percent ile documentation, counseling for nutrition, and counseling for physical activity” (CMS,
Explore Measures & Activities, 2022).
All three of the obesity quality measures are process measures; reporting began around 2009
(NCQA, Weight Assessment and Counseling for Nutrition and Physical Activity for Children/Adolescents,
2022) (NQF, Preventive Care and Screening: Body Mass Index (BMI) Screening and Follow up, 2020).
None of the obesity quality measures in use are outcome measures. The Adult BMI Assessment measure
was retired by NCQA in 2020 because of decreased relevancy due to automatic calculation, saturation of
measure reporting, and lack of reporting on the counseling and follow up component (NCQA, Proposed
Retirement for HEDIS MY 2020, 2020). NQF removed endorsement of the CMS Preventive Care and
