Recommendations for individualized pharmacotherapy based on weight-related complications are provided. Patients with a BMI ≥40 kg/m 2 without coexisting medical problems and for whom the procedure would not be associated with excessive risk should be eligible for bariatric surgery. Patients with a BMI ≥35 kg/m 2 and 1 or more severe obesity-related complications may also be considered for a bariatric surgery procedure. Patients with BMI 30-34.9 kg/m 2 with diabetes or metabolic syndrome may also be considered for a bariatric procedure. Obesity must be addressed consistently in clinical practice, as counseling by clinicians is a means to effectively assist in the weight management strategy of patients. Effective weight management interventions require a comprehensive lifestyle approach, one that integrates physical activity, nutrition, behavioral management, and gives attention to psychosocial needs. Provides recommendations based on the 5As counseling framework (Assess, Advise, Agree, Assist, and Arrange). Primary care physicians can use the 5As framework to build and coordinate a multidisciplinary team that: 1) addresses patients’ psychosocial issues and medical and psychiatric comorbidities associated with obesity treatment failure; 2) delivers intensive counseling that consists of goal setting, self-monitoring, and problem solving; and 3) connects patients with community resources to assist them in making healthy lifestyle changes. Review of current needs, recommendations for collaboration, and best practice examples for obesity prevention and treatment as it relates to the following areas: Provider Education and Training, Public Education and Activation, Research, and Policy and Advocacy.
