persistence with evidence-based treatment options, such as virtual weight management and anti-obesity medications?
Scale: Can integrated, well-targeted obesity treatment be delivered at scale? Is obesity a viable target for population health management and value-based provider payment models, or is obesity treatment better supported through payment models that are more specialty-focused, such as episode-based provider payment? Is appropriate billing infrastructure in place? What are the hurdles to validating quality metrics that will enable payers to pay for value and outcomes from integrated care? What is the role of value-based agreements?
The NEHI Roundtable
Facilitated discussion under the Chatham House rule NEHI proposes to convene a meeting of 15-20 experts for a facilitated discussion on the viability of obesity treatment as a goal for value-based provider payment and to identify concepts or models for coverage of obesity treatment that can be designed, tested, and implemented. The meeting will include consideration of changes in employees’ insurance benefits that complement provider payment models for obesity treatment, improve patient uptake of treatment and sustain patients’ adherence and persistence to treatment. Discussion at the meeting will be summarized by NEHI staff within a NEHI white paper and include recommendations for further action. To encourage as much candor as possible, discussion at the meeting will be held according to the Chatham House rule. No comments made during the meeting will be attributed to an individual. A draft of the NEHI white paper will be circulated for review by participants in the meeting before NEHI releases the paper publicly. Pre-read –NEHI “toolbox memo” As a preliminary step, NEHI will develop a pre-conference white paper that categorizes the analytical tools (e.g., for risk stratification), care models, digital health services, and anti-obesity medications s now emerging for active treatment of obesity, (i.e., the emerging “toolbox” of obesity prevention and treatment.) This memo will also be designed for general dissemination to NEHI member organizations, policymakers, health care leaders and the public. Focus on the role of self-funded payers Obesity and obesity treatment are complex subjects, and the future of obesity treatment is made more complex by the multiple types of health care payers and health insurance plans in the health care marketplace. NEHI’s view is that an initial discussion of the future of obesity treatment (the discussion proposed here) should be simplified by focusing on the role of one type of payer. Subsequent roundtables will consider the role of other payers and how a coherent strategy on obesity treatment can be created that aligns across all payers. NEHI proposes that the initial discussion focus on the role of self-funded health care payers, such as employers.
